Please enclose the following information along with your payment of $25 in the form of bank cashier’s check, money order, or personal check drawn on an Ohio bank made payable to the Wayne County Health Department:
1. Full name on certificate
2. Date of birth
3. Location of birth
4. Parents’ names, including mother’s maiden name
5. Name, address, and phone number of Requester
