Certified death certificates are available for $25 per for persons deaths that occurred in Wayne County after December, 1908.  (There is a $30 charge for each returned check.)

Please enclose the following information along with your payment of $25 in the form of bank cashier’s check, money order, or personal check drawn on an Ohio bank made payable to the Wayne County Health Department:

        1.    Full name on certificate

        2.    Date of death

        3.    Location of death

        4.    Name, address, and phone number of Requester
