Public Health Nurse 2

Immediate Supervisor:  Director of Nursing

Positions Supervised:  LPN


The Registered Nurse (RN) is responsible for managing the individualized patient care by promoting and restoring patients’ health through the nursing process; collaborating with physicians. Provides physical and psychological support to patients, friends and families; and supervising team members.


  • Graduate of an accredited school of Nursing
  • Must be a current Licensed Practical Nurse pursuant to Section 4723.01 of the Ohio Revised Code
  • Six month’s experience in public health nursing preferred, or equivalent nursing experience
  • Case Management Experience preferred
  • Valid driver’s license

  • Conducts an individual patient assessment, and develops a Plan of Care that is individualized for the patient reflecting collaboration with other team members of the healthcare team
  • Performs therapeutic nursing interventions as established by individualized plan of care for the patient and their families
  • Deliver care with appropriate age and cultural competence to specific patient populations according to individualized needs.
  • Interviews patient and records their medical history and physical condition
  • Obtains patient vital signs, including height, weight, blood pressure, pulse and/or temperature; obtains laboratory samples and readies these for testing
  • Apply nursing and public health principles to provide health services to individuals, families, groups and the community
  • Reviews medical chart
  • Assist Physician
  • Communicates with patient and their families
  • Administers immunizations according to current ACIP, ODH and health department policy; administered treatment as needed
  • Implements and Evaluates health programs (e.g. Project Dawn, VSP, TB)
  • Implements Communicable Disease programs, and initiates appropriate follow up while adhering to ODH Communicable Disease manual, assist with contact tracing
  • Conducts Health Promotion service programs in community settings (e.g. public schools, Amish Parochial schools, preschool programs and physician offices)
  • Presents Health Education, and coordinates/conducts health screening and services and completes all appropriate paperwork in a timely manner
  • Represents agency in a positive professional manner
  • Maintains current competency in nursing by attending educational workshops; reviewing professional publications
  • Participates in other nursing division and/or agency activities as assigned
