Certified birth/death certificates are available same day at the Wayne County Health Department for $25 per certificate for persons born in Ohio after 1908 or for deaths that occurred in Wayne County after 1908.

Birth Certificates
Please enclose the following information along with your payment of $25 in the form of a bank cashier's check, money order, or personal in-state Ohio check (out of state checks are not accepted) made payable to the Wayne County Health Department:
- Full name on the certificate
- Date of birth
- Location of birth in Ohio
- Parents' names, including mother's maiden name
- Name, address, and phone number of Requester
Death Certificates/Fetal Death Certificates
Please enclose the following information along with your payment of $25 in the form of a bank cashier's check, money order, or personal in-state Ohio check (out of state checks are not accepted) made payable to the Wayne County Health Department:
- Full name on the certificate
- Date of death
- Location of death
- Name, address, and phone number of Requester

Payment
The following forms of payment are accepted:
- Cash
- In-state Ohio check with driver's license/Ohio I.D. (Out of state checks are not accepted. There is a $30 fee for returned checks.)
- Credit card: There is an additional fee of 2.5% or $1.50 minimum for using credit cards.
