Certified birth/death certificates are available same day at the Wayne County Health Department for $27 per certificate for persons born in Ohio after 1908 or for deaths that occurred in Wayne County after 1908. There is a $2 processing fee to mail.

Birth Certificates
Please enclose the following information along with your payment of $27 (please note there is an extra $2 processing fee to mail your birth certificate) in the form of a bank cashier's check, money order, or personal in-state Ohio check (out of state checks are not accepted) made payable to the Wayne County Health Department:
- Full name on the certificate
- Date of birth
- Location of birth in Ohio
- Parents' names, including mother's maiden name
- Name, address, and phone number of Requester
Death Certificates/Fetal Death Certificates
Please enclose the following information along with your payment of $27 (please note there is an extra $2 processing fee to mail the death certificate) in the form of a bank cashier's check, money order, or personal in-state Ohio check (out of state checks are not accepted) made payable to the Wayne County Health Department:

Payment
The following forms of payment are accepted:
- Cash
- In-state Ohio check with driver's license/Ohio I.D. (Out of state checks are not accepted. There is a $30 fee for returned checks.)
- Credit card: There is an additional fee of 2.5% or $1.50 minimum for using credit cards.
